I have installed Asterisk 1.4 with TDM400P.

(a) It work flawlessly with a normal direct PSTN line from my Telco.

(b) When using a Centrex-based PSTN line from my Telco, the line is forced into off hook immediately (all incoming calls gets busy signal). This is the same with or without powering up the Asterisk PC, so it rules out all software issues.

(c) Tried with a few other Asterisk PC, different direct and Centrex-based lines, all yield the same result as (a) & (b) above.

(d) I was told the normal direct line is using standard 48V on hook volrage while the Centrex is providing a 24V on hook voltage.

Do I have a faulty TDM400P or it is known this card don't work with 24V on hook?

Appreciate any help.

I've confirmed this is due to the faulty TDM400P.

Replaced another card and everything is OK now.
