I'm looking to build a VoIP system for a clinic, 100-125 sets. They
currently have 2 PRI's. Would it be better to use a QUAD internal T1
card or use an external T1 gateway?

Cisco AS5300 is what we deploy typically. Or any other Cisco router
chassis with VFCs.
But really, this just gets into a religious war, particularly between
the higher and lower ends. You'll probably be hearing from Audiocodes
enthusiasts shortly, if anyone pipes up, and perhaps from the
considerably smaller and quieter but very adamant Lucent TNT segment.

Another option is a Mediatrix 3531.
One nice feature is that the ISDN diagnostics are really good.
Less than two K for a single T1 and Less than 3K for a dual T1.
